Structural Abnormalities Resulting in Infertility


Apart from physiological, immunological, and functional abnormalities that result in infertility, there are also some gross structural abnormalities in both men and women. Structural problems in women:
(1) Blocked fallopian tubes: Pelvic inflammatory disease in past or abdominal surgery may result in blocking of tubes in women. Structural problems in men:
(1) CAVD: Congenital absence of vas deferens results in inability to transport sperms into the semen.

(3) Undescended testicles: during fetal development if the testicles fail to descend from abdomen to scrotum, surgery needs to be done early in life to avoid infertility later.

Immunologic Conditions resulting in infertility


There are hoards of factors that are responsible for normal conception. Similarly there are different factors responsible for failure to conceive. The structural and functional causes can be identified pretty easily with various diagnostic tests. The immunologic factors however may be missed at times. It has been suggested that genital tract trauma, infections, varicocele, and vasectomy may precipitate such antisperm antibodies.
(2) Anti-phospholipid syndrome: Phospholipids are essential for proper embryo implantation. But when antibodies are formed against them, they lead to too early blood clotting in women, thus preventing the blood supply to baby.
(3) Pregnancy may fail due to mother developing antibodies to her own fetus’ DNA.
(4) High number of CD 19 or natural killer cells may damage the placental cells or the endocrine system necessary for the continuation of pregnancy.

Endocrine Dysfunction Resulting in Infertility


Among many problems that lead to infertility, endocrine dysfunction is one of the major ones. Also, recurrent pregnancy loss has been attributed to one of the endocrine dysfunctions in 10-25% cases. In women, fertility is well-regulated by hypothalamo-pituitary-ovarian axis. And the dysfunctions of thyroid, pancreas, adrenal glands, and pituitary gland disturb the normal HPO axis, resulting in pregnancy loss or infertility.

Following are the major endocrine disorders resulting in infertility:
(1) Hyper or hypo-thyroidism: moderate hyperthyroidism results in oligomenorrhea or amenorrhea and metrorrhagia. Severe cases lead to dysfunctions in LH and FSH, loss of midcycle LH peak, thus leading to anovulation and infertility. Also the risk of fetal growth retardation, intrauterine death, and pre-eclampsia are increased due to hyperthyroidism. Hypothyroidism usually results in anovulatory cycles and low FSH and LH. Pregnancy is rare with overt hypothyroidism and is associated with premature abortions, still births, neurologic dysfunctions, etc.
(2) Acromagaly: it results in decreased libido in both sexes and menstrual irregularities in females. Impaired gonadotropin secretion results in ovarian dysfunction and infertility.
(3) Diabetes: mortality rates in perinatal period remain high for undiagnosed and untreated diabetic cases, though insulin administration and therapy significantly reduces the risk. Menstrual irregularities and ovarian problems are common in diabetic women. Pre-implantation embryo development and folloculogenesis is affected in women with untreated diabetes. The fetal growth restriction is also common in diabetic mothers.
(4) Addison’s disease: the adrenal insufficiency results in amenorrhea and auto-immune ovarian failure, thus leading to infertility.
