Gestational Surrogacy – Picking the Right One

Due to advancement in gynecological and obstetric sciences, today couples enjoy better healthcare during their critical period in life. Especially those couples who fail to conceive despite opting for the best possible treatment like to opt for surrogacy. The taboo surrounding surrogacy is fast nullifying and many couples are coming forward to have a baby through gestational surrogacy. In this type of surrogacy, baby is conceived entirely by the intended parents. The father’s sperm fertilizes mother’s egg in laboratory setting and the resultant embryo is placed into the uterus of surrogate mother and is carried to term.

Apt legal help is also provided by standard hospital set ups like Palm Beach Fertility Center. The surrogate mother should be highly committed woman under 40 with at least one healthy child of her own. Proper medical and psychological testing is done before finalizing a surrogate. The selection process is very crucial for successful and hassle-free delivery of baby.

The doctors often make this woman undergo a mock cycle during which her uterine lining is checked by administering fertility hormones to her. Mock transfer is also carried out in which the cervical angle and uterine length is measured. What couples need is apt support and psychological counseling while going through this phase. Only expert and compassionate doctors can impart firm support and assurance to such couples. Therefore, it is well said that though choosing the surrogate mother is very important, first you must choose a good hospital and good doctor who will ensue everything smoothly for you!

Structural Abnormalities Resulting in Infertility

Apart from physiological, immunological, and functional abnormalities that result in infertility, there are also some gross structural abnormalities in both men and women. Rather, when a couple consults the physician for the first time, foremost thing the physician should do is evaluate the structural normalcy in both the partners.

Structural problems in women:
(1) Blocked fallopian tubes: Pelvic inflammatory disease in past or abdominal surgery may result in blocking of tubes in women. Obviously the blocked tubes interfere with the fertilization process and also do not allow the embryo to implant properly. Hysterosalpingography or laparoscopy may have to be done to investigate the problem and treat accordingly with the help of a specialized surgery.
(2) Uterine abnormalities: Gross abnormalities include double uterus or even lack of uterus. In such cases, one has to rely upon surgery if possible as in case of doult uterus or surrogacy if the uterus is absent altogether.
(3) Cervical mucus problems: During conception period, the usually thick cervical mucus gets less acidic and elastic for normal penetration of sperm. But if it remains thick, it can impede sperm entry resulting in infertility. If nothing else helps, you might have to undergo IUI for fertilizing egg properly and ensuing pregnancy.

Structural problems in men:
(1) CAVD: Congenital absence of vas deferens results in inability to transport sperms into the semen.
(2) Hypospadias: instead of being on the tip of the penis, the urethral opening is on the underside of the penis. Depending upon the relation to infertility, surgery is done.
(3) Undescended testicles: during fetal development if the testicles fail to descend from abdomen to scrotum, surgery needs to be done early in life to avoid infertility later.

Immunologic Conditions resulting in infertility

There are hoards of factors that are responsible for normal conception. Similarly there are different factors responsible for failure to conceive. The structural and functional causes can be identified pretty easily with various diagnostic tests. The immunologic factors however may be missed at times. Varied stages of proper conception are affected due to faulty immune system. Either patient presents with unexplained infertility, recurrent abortions or recurrent IVF failures.

The natural killer cells or CD 56 cells produce toxic cytokines that in turn produce antibodies to hormones like HCG and progesterone. Thus because of this abnormal response of the body to produce antibodies to its own helpful cells, problem ensues. There are different causes for this:

(1) One or both partners may be having antisperm antibodies. This makes the active sperm ineffective and unable to fertilize the egg. Sometimes these antibodies are present in the ejaculate itself thus preventing the sperms from releasing effectively. It has been suggested that genital tract trauma, infections, varicocele, and vasectomy may precipitate such antisperm antibodies.
(2) Anti-phospholipid syndrome: Phospholipids are essential for proper embryo implantation. But when antibodies are formed against them, they lead to too early blood clotting in women, thus preventing the blood supply to baby.
(3) Pregnancy may fail due to mother developing antibodies to her own fetus’ DNA.
(4) High number of CD 19 or natural killer cells may damage the placental cells or the endocrine system necessary for the continuation of pregnancy.

Advanced tests and investigations in standardized state of art laboratories have made it possible to identify even some of the immunologic abnormalities so that appropriate steps can be taken to curb future menace!

Endocrine Dysfunction Resulting in Infertility

Among many problems that lead to infertility, endocrine dysfunction is one of the major ones. Also, recurrent pregnancy loss has been attributed to one of the endocrine dysfunctions in 10-25% cases. In women, fertility is well-regulated by hypothalamo-pituitary-ovarian axis. And the dysfunctions of thyroid, pancreas, adrenal glands, and pituitary gland disturb the normal HPO axis, resulting in pregnancy loss or infertility.

Following are the major endocrine disorders resulting in infertility:
(1) Hyper or hypo-thyroidism: moderate hyperthyroidism results in oligomenorrhea or amenorrhea and metrorrhagia. Severe cases lead to dysfunctions in LH and FSH, loss of midcycle LH peak, thus leading to anovulation and infertility. Also the risk of fetal growth retardation, intrauterine death, and pre-eclampsia are increased due to hyperthyroidism. Hypothyroidism usually results in anovulatory cycles and low FSH and LH. Pregnancy is rare with overt hypothyroidism and is associated with premature abortions, still births, neurologic dysfunctions, etc.
(2) Acromagaly: it results in decreased libido in both sexes and menstrual irregularities in females. Impaired gonadotropin secretion results in ovarian dysfunction and infertility.
(3) Diabetes: mortality rates in perinatal period remain high for undiagnosed and untreated diabetic cases, though insulin administration and therapy significantly reduces the risk. Menstrual irregularities and ovarian problems are common in diabetic women. Pre-implantation embryo development and folloculogenesis is affected in women with untreated diabetes. The fetal growth restriction is also common in diabetic mothers.
(4) Addison’s disease: the adrenal insufficiency results in amenorrhea and auto-immune ovarian failure, thus leading to infertility.

Well, the major thing one must remember is that with any of the endocrine dysfunctions, you need to get treated as early as you can after careful assessment of condition by expert doctors. You definitely have chances of conception thereafter!

Chromosome Abnormalities

Sex chromosomes are the major chromosomes involved in the abnormal presentations. The chromosomal abnormalities that are autosomal in origin are slightly less common. The autosomal disorders are however much less severe in their effects. The sex chromosomes abnormalities are gender specific. When there is variation in the number of X chromosomes, female fetuses suffer from chromosome abnormalities, whereas, irregular numbers of either X or Y chromosomes or both result in male abnormalities.

The major chromosomal abnormalities are:
Turner syndrome: in which the females inherit only single X chromosome.
Triple X females or metafemales: in which females inherit more than two X chromosomes.
Klinefelter syndrome: in which males inherit extra X chromosomes.
XYY syndrome: in which the males inherit an extra Y chromosome.

Also the other genetic disorders like cystic fibrosis, sickle cell anemia, Huntington disease, hemophilia, Tay-Sachs disease, and certain leukemias also result due to some or the other chromosomal abnormality.

This is the reason medical experts suggest the PGD test at much earlier stage. This is nothing but Pre-implantation Genetic Diagnosis. Especially the parents whose progeny is at increased risk of inheriting genetic disorder can be helped through this test. After the completion of IVF cycle, one cell from each of the fertilized embryos is tested form DNA mutation that may be considered responsible for a particular genetic disease. This test also helps diagnosing gender of the baby.

That is why you should always search for the right hospital in which such advanced investigations are performed in the state of art laboratory and by expert staff!

New Therapies for Recurring Pregnancy Loss

It is very natural for a couple to get tensed after recurrent pregnancy loss or miscarriage. In the first place, they do not understand what is wrong and with whom it is wrong. Whoever is at fault, what the couple needs is apt investigations in a state of art laboratory and apt friendly counseling support from their doctor. After this comes the role of apt treatment. Yes, it is well said that good counseling helps enhance the compliance of the couple for appropriate treatment.

The treatment plan should obviously be based upon the causative factor where it can be figured out. However, the risk of increased pregnancy loss enhances with the increasing age of mother and father, because this results in genetic mutations of genes and chromosomes. Genetically balanced rearrangement of chromosomes in one of the partners can result in recurrent pregnancy loss. In such cases the couples may have to opt for in-vitro-fertilization.

Surgery is indicated where the woman is having physical problems with her uterus, either structural or due to endometriosis or fibroid. Women in whom Anti-phospholipid syndrome is the cause of recurrent miscarriage, heparin as a blood thinner may be given for successful pregnancy outcome. Many doctors however still believe in progesterone therapy for recurrent miscarriages. A sustained level of progesterone help hold the pregnancy is true according to many experts.

Immunotherapy is the newer technique to treat recurrent pregnancy loss, however, exact mechanism how it works and whether it can be used in every individual case are the things that need more research. The final decision depends upon the expert doctor. So, getting in touch with the right doctor is the foremost thing one should strive for!

Miscarriages – When to try again

It is true that not all couples are fortunate enough to have first normal conception that they can carry to term. Rather there is more number of women who have miscarriages in first trimester with or without a known reason. Those women often consult a doctor for taking a second or next chance after their initial miscarriage. And it is obviously right to opt for an obstetrician and gynecologist before you take any decision, as each case may vary from the other and therefore, it is better not to depend upon any of your friend’s advice.
Also it is evident from numerous studies that either of the following may be advised to couples trying to conceive after miscarriage:
(1) Leave a gap of 6 months: Especially the woman who undergoes D&C has to wait little longer that those with complete abortion. This is because little more time (4-6 months) may be required for the endometrial lining to replenish after early miscarriage.
(2) No need to wait: Many doctors believe that after miscarriage, the body takes care of the replenishing part pretty soon and till the time woman conceives again, she is fit to carry on pregnancy without any hassles. So, doctors may advise you to start trying earlier.
(3) Let your normal period begin: Well, some doctors believe that when a woman conceives immediately after miscarriage, without having had her period in between, it is difficult to guess exact duration of pregnancy in initial stages. Also, the doctors believe that normal ovulation occurs only after the first normal natural period following miscarriage. Therefore, to conceive healthily it is better to wait till the woman has her first menstrual cycle after miscarriage.

Your choice of a good gynecologist and obstetrician goes a long way in establishing firm footing for your further course of treatment. No wonder people like to opt for popular and promising names in these matters, so that they have someone to depend upon. Once entered the doctor’s office, you are ought to follow what he or she says.
Different doctors have different types of treatment options for fertility ranging from medicines, surgery, and assisted reproductive techniques. Obviously, the first step after confirming the initial investigations is medicine or fertility drug. There are different types of fertility drugs available in market today. However, it is important that you get yourself checked regularly after starting medicines for knowing whether your drugs are acting properly and not giving rise to any adverse conditions like ovarian hyper stimulation syndrome.
The women may be prescribed following drugs:
(1) To induce ovulation: She may receive bromocriptine, GnRH, tamoxifen, Clomifene or gonadotropins in the form of subcutaneous or intramuscular injections.
(2) To help your ovaries produce mature eggs, you may receive different drugs depending upon variety of reasons like HCG, Progesterone, Urofollitropin, Heparin, HmG, and/or recombinant FSH.
(3) Both men and women are benefited by a newer range of drugs called aromatase inhibitors, which work as alternative hormone therapy for both.
Here are certain brand names of popular fertility drugs:
(1) Bromocriptine: Parlodel
(2) Clomifene: Serophene, Milophene, Clomiphene, Clomid, etc.

Marijuana & Male Sterility

When doctors check infertile couples, they always lay more stress on the lifestyle of these couples. They take detailed history of past and present lifestyle and habits. It is because not always people face grave problems like infertility due to biological problems. Many times, their bad habits land them in trouble. Similarly, marijuana smokers are said to have low fertility ratios that those who do not smoke marijuana.
Marijuana contains active chemical THC (tetrahydrocannabinol). Though it is not evident still how this THC affects sperms, two postulates have been laid in this connection:
(1) THC might be interrupting the good timing of sperm function.
(2) It may also be bypassing the natural inhibition mechanisms.
Because of these two factors, sperms move too fast and too early than desired for proper conception. During many other studies, it was also observed that THC led to abnormalities of the sperm cap or acrosome. The normal swimming patterns were changed and there was also reduction in the ability of sperm to attach to egg for fertilization.
From some studies, it was also evident that marijuana smokers had less seminal volume and also less sperm count that those who do not smoke. Especially those men who have borderline fertility ratio are more likely to be pushed into infertility zone by smoking marijuana.
Also it is evident that those women who smoke marijuana are less likely to reach to term with their pregnancy. Especially women who smoke during early pregnancy or during the time of conception have problems with embryo’s safe passage from ovary to uterus, thus resulting in pregnancy failure. No wonder doctors advice both men and women against the marijuana smoking. After all, you want to have normal conception and a healthier offspring!

Medicines that Help One get Pregnant!

No doubt conception is a natural process and most couples conceive after 1 year of unprotected sex. But if they fail to conceive during this period it is necessary that they get investigated themselves for the problems and take appropriate treatment from expert doctors to make conception happen. Obviously the treatment depends upon the cause of failure to conceive.
If the problem lies with woman and she suffers from ovarian dysfunction, she has to be treated for the same. Medicines stimulating ovulation like clomifene may help these women. If the problem lies with egg maturation, still the same medicine may help her have normal conception after careful ultra-sonographic monitoring.
It is said that in many women, doctors give initially only folic acid in an attempt to have natural conception, especially if there is no other abnormality found. Yes, folic acid is the supplement that has given amazingly satisfactory results over the years and even the neural tube defects are minimized significantly in the fetus after using folic acid regularly in the initial trimester.
If the problem lies with male partner, obviously the doctor has to give him the necessary sperm stimulating drugs that will enhance his semen volume as well as healthy sperm count. The lycopene supplements are said to act wonderfully in this regard to achieve pregnancy. Certain homeopathic medicines are also said to work effectively to enhance sperm function.
Thus, one has to work towards treating the causative factor while trying to conceive. Do not hesitate to seek advice from expert doctors and never self-medicate in this connection. The doctors like those in Palm Beach Fertility Center will be pleased to inform you after careful assessment of your case!

Pros & Cons of Progesterone in Hormone Replacement Therapy:

Woman undergoes many milestones in her life and menopause is one of them. This is the phase when she is physically mentally, and emotionally disturbed due to various hormonal changes that take place in the body. The normal monthly cycles come to a standstill and her estrogen and progesterone levels drop down, thus resulting in different problems ranging from hot flushes, vaginal dryness, frequent fractures, and osteoporotic bone lesions. No wonder many investigations and studies are going on to study the efficacy of hormone replacement therapy to avoid these problems during menopause. But it is true that not all women can undergo hormone replacement therapy just for the sake of it. The physician has to assess the status of woman before commencing on either estrogen therapy, progesterone therapy or a combination therapy. Let’s see pros of progesterone therapy: (1) The risk of colon cancer is lowered significantly with progesterone therapy. (2) The cases of hip fracture are reported far less with progesterone therapy. (3) Progesterone can be said as the most effective treatment for treating hot flushes and vaginal dryness and itching. (4) The combination therapy containing estrogen and progesterone is capable of reducing the heart disease risk in these menopausal women, though it is also suggested not to commence this treatment just for the sake of reducing heart disease risk unless there are other complications of menopause existing along with. Cons of progesterone therapy: (1) Patient may suffer from bloating or enhanced irritability (2) Long term use may increase the risk of breast cancer (3) In women having problems of blood clotting, this therapy might be dangerous (4) Risk of stroke is also known to enhance. Therefore, only after careful assessment from an expert doctor one should commence with HRT that starts at minimal dosage!

Wet Heat Contraception

Dr. Voegeli was the man who taught the wet heat contraception method to Indian men during famines. His method of natural contraception though little controversial, gained much momentum and even today there are such practices that can give you guaranteed results without necessity of gulping any tablets or pills.
No wonder people were in love with this contraceptive method because of its natural formulation and result-oriented aptness. According to Voegeli, the man sits in hot water bath of 116 degrees Fahrenheit for 45 minutes per day for 3 weeks. After this it was evident that he is sterile for the next six months. If one wants to continue the method, he can do so by increasing the duration of wet heat application. It was also evident that after this method of contraception, normal fertility resulted after 6 weeks unless the man wants to enhance the period of contraception.
Many researches have been carried out in this area and also many are still going on in different parts of United States and the rest of the world. Instead pf hot water, heat packs or heating pads are used. Even with these newer methods, the sperm count bounces back quite effectively after stopping the earlier wet heat method, unless they are active smokers that encountered problems later.
But there are scientists who believe that the wet heat method to be effective and reliable as contraceptive, higher temperatures may have to be used and therefore, this cannot be recommended on regular basis to all people. However, for those people who loathe other methods of contraception and have limitations in trying out other options, this may be one of the only ways for maintaining control over their fertility.
