
Apart from physiological, immunological, and functional abnormalities that result in infertility, there are also some gross structural abnormalities in both men and women. Rather, when a couple consults the physician for the first time, foremost thing the physician should do is evaluate the structural normalcy in both the partners.
Structural problems in women:
(1) Blocked fallopian tubes: Pelvic inflammatory disease in past or abdominal surgery may result in blocking of tubes in women. Obviously the blocked tubes interfere with the fertilization process and also do not allow the embryo to implant properly. Hysterosalpingography or laparoscopy may have to be done to investigate the problem and treat accordingly with the help of a specialized surgery.
(2) Uterine abnormalities: Gross abnormalities include double uterus or even lack of uterus. In such cases, one has to rely upon surgery if possible as in case of doult uterus or surrogacy if the uterus is absent altogether.
(3) Cervical mucus problems: During conception period, the usually thick cervical mucus gets less acidic and elastic for normal penetration of sperm. But if it remains thick, it can impede sperm entry resulting in infertility. If nothing else helps, you might have to undergo IUI for fertilizing egg properly and ensuing pregnancy.

(1) CAVD: Congenital absence of vas deferens results in inability to transport sperms into the semen.
(2) Hypospadias: instead of being on the tip of the penis, the urethral opening is on the underside of the penis. Depending upon the relation to infertility, surgery is done.
(3) Undescended testicles: during fetal development if the testicles fail to descend from abdomen to scrotum, surgery needs to be done early in life to avoid infertility later.
